Repair Order Form

    Please fully read and understand the following terms before submitting your personal property or belongings with Prismatic Technology.

    For water or fire damaged devices we only carry out data recovery. Liquid damage can have long-lasting effects; therefore, this is not covered under our warranty.

    Prismatic Technology is not liable for any data loss. Please back up your device before sending it in.

    (Routine repairs): Moves you to the front of the line; often same-day but not guaranteed. Fee is non-refundable. (Data recovery): Priority Data pass to accommodate faster attempts at recovering data this fee is J$15,000, non-refundable. Priority fees are non-refundable and do not guarantee same-day completion.

    Remove your case, SIM card/tray, and memory card before submission. Prismatic Technology is not responsible for loss of these items.

    If payment is not received within 25 days after invoice date, you agree to forfeit your device in lieu of payment and Prismatic Technology reserves the right to recycle it. Estimate valid for 30 days.

    Default policy: 50% of estimate + full allowed rush fee, rounded up to the nearest J$250. Minimum deposit is J$30,000 for Data Recovery and J$8,000 otherwise.





    Jobs with prior micro-soldering have increased complexity and a reduced likelihood of success; the additional fee covers the extra time and risk.

    Liquid/fire damaged devices are handled as data recovery only and are not covered under warranty.

    We cannot accept responsibility for features we could not test prior to repair. Our no-fix = no-fee policy covers the primary problem only; secondary defects fall outside this policy.

    Warranty & Terms
    Warranty: All board-level repair services are guaranteed by our industry-leading lifetime warranty. Our guarantee covers the primary issue(s) repaired, not the entire device. Accidental liquid, charger, or drop damage after collection is not covered. If your repair fails within three months and we cannot correct workmanship, we refund the repair cost. Beyond three months, if we cannot correct workmanship, we issue store credit for the repair cost. Complimentary repairs are case-by-case and are not covered under warranty.

    Water Damaged: For water- or fire-damaged devices we perform data recovery only. Long-term reliability cannot be guaranteed and is not covered under warranty.

    Unpaid Invoices: When we complete a repair, an invoice is generated. Pay in full within 21 days. Invoices not paid in full within 25 days incur an $8,000 storage fee. We hold devices and unpaid invoices for no more than 90 days. By signing, you understand that after 90 days you relinquish ownership of any items listed on this document or left in our possession to Prismatic Technology.

    Return / Refund Policy: If something we fixed does not stay fixed, return the device for rework. If we cannot repair it, we refund the repair. We cannot refund for issues unrelated to our repair. Shipping charges are not refundable.

    Diagnostic Fee: Diagnostic services are not included in the repair cost. If a diagnosis identifies a solution but you choose not to proceed, we charge a diagnostic fee equal to 50% of the quoted repair price.
